The last team to do it was Indiana in the year '76.  Interestingly enough, the National Championship will be in Indianapolis this year, where Kentucky has a chance to match the accomplishment of the Hoosiers from 39-years earlier.

With regards to '76, that is the same year Ebola was discovered.
